The Patient-Centered Medical Home (PCMH) is a model of care that puts you at the forefront of care. PCMH is centered on improving relationships between you and your provider team to improve your care.

Research shows that PCMH practices improve quality of care and enhance the patient experience – all while reducing health care costs. Being a PCMH practice, Nasson is committed to continuous quality improvement and a patient-centered approach to care that is evidence-based and delivered in accordance with best practices.

  • The model is team-based and led by your chosen Primary Care Provider
  • Your team includes Medical, Dental and Behavioral Health providers – as well as any care management or support services you might require.

YOUR CARE

Your team of providers and our staff are here to help you feel better, set your personal health goals and create pathways to achieve them. As a patient at Nasson you can expect the following:

  • Patient-Centered: Our care model and how we are organized is centered around meeting your care needs, being flexible enough to accommodate your preferences and to respect your values. We also look for ways to actively engage you in your care.
  • Comprehensive: We offer a wide array of preventative, acute and chronic care services. Your Care team is here to help you set your personal health goals and create a care plan to achieve them. We will answer your questions, provide sound advice, and make sure you understand any health risks or conditions you may have – as well as treat and manage them.
  • Coordinated: Our providers will help you access the clinical services you need whether they are referrals to specialty care, hospital services, medical procedures or diagnostic tests. We will also help coordinate appointments and follow up with your providers after your visit.
  • Accessible: Our Health Center is open early morning and evenings during the week so that you can schedule appointments that fit your lifestyle. For those times when care can’t wait, we also provide walk-in hours and a limited amount of same day appointments. Additionally, we offer an after-hours triage service where you can call our main number and a nurse will assist you with medical advice and access to an on-call provider if deemed necessary. Patients also have access to their provider by using our patient portal app to send them a message and seek assistance.
  • Improved Quality: The care we provide is evidence-based and delivered in accordance with best practices by highly qualified and experienced providers that are here for you.
  • Respectful and Confidential: Your information will never be shared without your permission. That includes anything you discuss with your provider during your visit, any information or referrals you receive, and any concerns you express related to your health.

YOUR ROLE

You are the most important member of your care team. While your providers can act as coaches and resources, you are the key player in achieving better health. The following are ways you can help us create the most effective care plan for you:

  • Communicate with us: We want to know about your health, your social needs and what you would like to accomplish with your care. Be sure to let us know about changes in your condition, your medications, or if you receive care from another provider, specialist, hospital or urgent care facility.
  • Follow your Care Plan: You will work with your clinical team to develop a personalized health and wellness plan that fits your lifestyle, meets your needs and helps you achieve your health goals. Complete any recommended test and take your prescribed medications as instructed.
  • Prepare for your visit: Try to keep your scheduled appointments and let us know in advance if you need to reschedule or if you need help with transportation. Bring all requested documents (list of medications, insurance information, etc) and tell us if any of your information on file has changed (contact, insurance, etc).
  • Stay informed: Ask questions about your health and address any concerns you might have with your providers about your care plan or how you are feeling. Our team is here to ensure you have the right plan to get better and achieve your health goals.

AFTER YOUR VISIT

We will provide you with an after-visit summary at the end of your appointment and assist you in setting up any referrals or specialty appointments you may require. Our team will always respond to any questions or concerns you may have in a timely manner. We will also deliver test results using the method you select (phone, email, mail or other means).
